What are the side effects of tirzepatide?
Mostly stomach problems, plus a few rarer but serious warnings.
In the Zepbound label trials, nausea affected 25-29% of people versus 8% on placebo, diarrhoea 19-23% versus 8%, and vomiting 8-13% versus 2% . Between 4.8% and 6.7% stopped treatment because of side effects, depending on dose .
Label warnings include:
Thyroid C-cell tumours, a boxed warning based on rat studies. It is contraindicated with a personal or family history of medullary thyroid cancer or MEN 2 . Whether this happens in humans has not been determined .
Pancreatitis, gallbladder disease, and kidney injury from fluid loss .
Severe stomach and bowel reactions. It is not recommended in severe gastroparesis .
Serious allergic reactions, including anaphylaxis and angioedema .
Low blood sugar when combined with insulin or sulfonylureas .
Worsening diabetic retinopathy .
The common side-effect figures come from large randomised trials of the branded drug, so they are strong evidence. The rarer serious risks rest on label warnings and animal work, so they are less certain. Unbranded "research use only" tirzepatide is not FDA-approved, and the FDA has treated such products as unapproved new drugs . The side effects of those products have not been studied.
Tell a prescriber promptly about severe or persistent abdominal pain, especially pain that spreads to the back, and about any swelling or breathing trouble.
